收集個人資料的注意事項：
Important Notes for collecting personal data

1. 申請人/戶如在申請PH2時作出虛假陳述或提供虛假資料，不論有關虛假陳述/虛假資料對其PH2申請資格是否構成影響，PH2均可取消該已登記的PH2申請，並保留一切法律追究權利。PH2亦可終止藉虛假陳述/虛假資料而獲得優先入住PH2的租約。
   If the Applicant(s) make false statement or furnish false information when applying for the PH2, irrespective of whether the false statement/false information has any impact on the eligibility of the application, the PH2 may cancel the registered application and reserve all rights to take further actions. The PH2 may also terminate the tenancy agreement acquired through making false statement / furnishing false information.
2. 申請人/戶應細閱及確定符合所有申請資格才遞交申請。申請人/戶必須清晰、詳盡、準確和如實地填報申請表及相關的聲明書。如申請人/戶在申請時並未符合有關資格，其獲登記的申請即屬無效並會被取消。
   The Applicant(s) should read all the eligibility criteria carefully and ensure that all the eligibility criteria are fulfilled before submitting the application. The Applicant should provide clear, full, accurate and true information to the application form and relevant declaration form(s). If the Applicant is not eligible at the time of application, his/her registered application will be invalid and cancelled.
3. PH2在審核申請的過程中，可要求申請人/戶及名列於申請表內的家庭成員提供有關的個人資料。若所提供的資料不足夠，PH2可能無法辦理該宗申請個案。
   In the course of vetting an application, the PH2 may request the Applicant and the family members listed on the application form to provide their personal data. Failure to provide sufficient information will render the PH2 unable to process the application.
4. PH2在處理申請時，可向有關的政府部門，公營/私營機構(例如金融機構及銀行)及/或任何擁有申請人/戶個人資料的第三者(例如僱主)蒐集個人資料進行核對，以核實申請資格。在蒐集資料過程中，PH2可將申請人/戶的個人資料向上述機構/或第三者披露。同時，申請人/戶亦須授權上述機構及/或任何擁有其個人資料的第三者向PH2提供其個人資料，以核實其PH2申請。
   The PH2 may, in processing the PH2 application, collect the personal data of the Applicant and his/her family members from other relevant government departments, public /private organisations (such as financial institutions and banks), and/or any other third party (such as employers) in possession of such personal data for verification and confirmation of their eligibility. In the process of data collection, the personal data of the Applicant and his /her family members may be disclosed to the above-mentioned organisations and/or any other third party. In this connection, the Applicant and his/her family members shall authorise these organisations and/or any other third party in possession of their personal data to furnish the PH2 with such data for vetting the application.
5. PH2不時與房委會及房屋署聯繫，將成功獲配並簽約入住PH2單位的申請人/戶的個人資料向上述機構/或第三者披露，以核對租戶已向房委會及房屋署申報其入住PH2的情況，及沒有違反相關享用資助房屋的條例，並就此向房委會及房屋署申請「暫時遷離」公屋及任何資助房屋。
   Upon allocation of the PH2 units, the PH2 will transfer the relevant personal data and all correspondences relating to application to the Hong Kong Housing Authority and Housing Department for the purpose of implementation of public housing policy, and to ensure the application of the temperately absent of the Public Rental Housing and Interim Housing was made by the PH2 tenant.
6. PH2亦可能使用申請表所提供的資料進行統計調查或研究，並就此聯絡申請人/戶。
   The PH2 may use the information contained in the application form for statistical survey or research, and approach the Applicant and the family members listed on the application form.
7. 每份申請將收取$100申請費，於獲邀面談時繳交，申請成功與否，均不會發還。若有人藉詞給予方便索取利益，或登門造訪申請人，藉詞協助PH2申請而索取金錢，申請人應立即向廉政公署舉報。PH2職員均持有由香港青年協會發出的職員證，申請人應要求查閱，才回答任何查詢。任何人意圖行賄，亦屬違法，PH2會將個案轉介廉政公署查究，不論是否因此被起訴或定罪，PH2均可取消其申請。另外，申請人亦不必聘請專業顧問協助辦理申請事宜，此舉不會令申請獲得優先處理。
   Each application will be charged a $100 application fee, which must be paid upon invitation for an interview. The application fee is non-refundable regardless of the success of the application. The Applicant(s) should report to the Independent Commission Against Corruption (ICAC) immediately in case anyone offers to provide assistance in return for remuneration or pays a home visit to the Applicant(s) and offers to assist in the application in return for remuneration. Every staff member of the PH2 carries a staff card issued by the PH2, and the Applicant(s) should request an inspection of it before responding to any questions. Attempted bribery by any person is also an offence in law, the PH2 will refer the case to ICAC for investigation and cancel the application, irrespective of whether such person has been prosecuted for or convicted of the offence. Besides, the Applicant(s) need not hire a professional consultant to handle the application. This will not accord any priority to the application.
8. 在申請表內所填報的個人資料，是為PH2的申請而提供。根據《個人資料（私隱） 條例》（第 486 章），申請者有權要求查閱或改正申請表上所申報有關其本人的資料。如有需要，須以電郵方式提出申請，電郵為(ph2@hkfyg.org.hk)。申請查閱個人資料，可能需繳付費用。
   The personal data in the application form are furnished to the PH2 for the purpose of PH2 application. Pursuant to the Personal Data (Privacy) Ordinance, the Applicant and his / her family member(s) are entitled to request access to or correction of the personal data declared in this application form. Where necessary, such requests should be made in writing and directed by email (ph2@hkfyg.org.hk). A fee may be charged for the request for access to personal data.

1. 我/我們在填報申請表前，已詳細閱讀及明白「申請須知」所載各項政策及規定的內容。我/我們承諾將會遵守相關的規定/安排及香港青年協會因應情況訂定或修訂的PH2申請及安排；
   I/We have read carefully and understood all the content of the “Application Guide” before completing the application form. I/We undertake to comply with the relevant requirements/arrangements set out by The PH2 as required;
2. 我/我們在填寫申請表當日並無擁有、與他人共同擁有或簽訂任何買賣合約購買載列在「申請須知」內相關條文所包括的各種香港住宅物業或持有任何直接或透過附屬公司擁有香港住宅物業的公司50%以上的股權；
   As at the date of completing my/our application form, I/we have not owned, co-owned, entered into any agreement to purchase or held more than 50% of shares in a company which owns (directly or through its subsidiary) any domestic property in Hong Kong covered in the relevant term of the “Application Guide”;
3. 申請表所列人士均未重複名列在其他PH2的一般申請內；
   No person listed on this Application Form is duplicated in other PH2 application(s);
4. 倘若我/我們的申請資料有任何改變，必須立即以書面通知PH2。我/我們承諾將會應PH2的要求，在指定的期限內提交所需的資料/證明文件，否則PH2會因為無法處理我/我們的個案而可能取消我/我們已登記的PH2申請；
   If there is any change in the information declared concerning the application, I/we shall immediately notify The PH2 in writing; otherwise, The PH2 may cancel my/our registered application as it cannot be processed further;
5. 我/我們明白申請PH2是沒有性別限制，女士或男士均可成為申請人並擁有同等權利和責任；
   I/We understand that there is no restriction on the sex of the applicants. Both male and female are eligible applicants with equal rights and responsibilities;
6. PH2為審核我/我們的申請，可向有關的政府部門、公營/私營機構（例如但不限於金融機構及銀行）及/或任何擁有我/我們個人資料的第三者（例如但不限於僱主）蒐集我/我們的個人資料進行核對，以核實我/我們的申請資格。在蒐集資料過程中，PH2可將我/我們提供的個人資料向上述機構及/或第三者披露。同時，我/我們亦授權上述機構及/或任何擁有我/我們個人資料的第三者向PH2提供我/我們的個人資料，以核實我/我們的申請；
   I/We agree that The PH2 may, in processing my/our application, collect my/our personal data from other relevant government departments, public/private organisations (such as but not limited to financial institutions and banks), and/or any other third party (such as but not limited to employers) possessing my/our personal data for verification and confirmation of my/our eligibility. Whilst the collection of information is in progress, the personal data contained therein may be disclosed to the abovementioned organisations and/or any other third party. In this connection, I/we authorise these organisations and/or any other third party possessing my/our personal data for the vetting of my/our application;
7. 我/我們同意，PH2可使用本申請表所提供的資料進行統計調查或研究，並就此聯絡我/我們；
   I/We agree that the information provided in this Application Form may be used by The PH2 for statistical survey or research, and I/we may be contacted for such a purpose;
8. 本申請表內所填報的事項，全部屬實，正確無訛。我/我們明白，任何人於申請PH2時明知而作出虛假陳述，即屬違法。我/我們如作出虛假陳述或提供虛假資料，藉以獲編配PH2單位，不論是否因此被起訴或定罪，PH2均可取消我/我們的申請及終止我/我們的有關租約。
   All the above particulars furnished in this Application Form are true and correct. If I/we make false statement or furnish false information when applying for the PH2, irrespective of whether I/we shall be prosecuted for or convicted of the offence, or whether the false statement/false information has any impact on the eligibility of the application, The PH2 may cancel the registered application and terminate the tenancy;
9. 我/我們明白及同意申請入住PH2的同時，將自動加入成為「青協會員及青協義工」，並受有關條款及細則約束。
   I/We understood and agree that I /we will be registered as HKFYG Member(s) and volunteer(s) automatically through this application, and understood the terms and conditions of HKFYG Membership.
